SURGICAL INSTRUCTIONS
WHAT TO EXPECT
Your appointment will consist of a consultation explaining your diagnosis and treatment options. Most usually treatment can be done the same day as the consultation.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
Please assist us to ensure your appointment goes smoothly by completing these steps:
Prior to your appointment:
Please fill out patient paperwork through our online portal (patient online forms) which will submit directly to our office. If you are a returning patient we will ask that you complete the online paperwork if it has been longer than one(1) year since you have been in the office.
Complete the insurance portion of these forms in its entirety (in the patient information section of the online forms) if you would like our office to be able to help you with the estimated cost of your visit.
Please bring with you to your appointment:
- Referral from your general dentist or any paperwork that your dentist has given you regarding your appointment
- Dental insurance card
- Drivers license or photo ID card
- Form of payment for you copay, which is due at your time of service
- Headphones if you would like to use during your procedure
Day of your appointment:
Every patient will have x-rays once you are taken back by our assistants. Our staff will take a CBCT scan and chairside x-rays prior to the examination.
X-RAYS
If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office.
